Case Management Services
Case Management Services - Meals on Wheels of Rhode Island’s case managers provide services to over one thousand elderly and adults with disabilities clients per year. The Case Management Department provides a full range of social services for Providence’s residents 60 years of age and older. Through case management services, clients receive an assessment of their needs. A care plan is developed which includes a variety of options that allow older adults to remain in his or her own home. The case manger will monitor the care plan and offer support for the elder and his or her family members.
The case manager works closely with the RI Department of Elderly Affairs. (RIDEA)
Services include:
Home And Community Care Program - based on eligibility, this program provides seniors with personal and home care. To be eligible, a person must be age 65 and over, reside in the city of Providence and have a level of care that shows the client needs homemaker services.
Assisted Living Waiver Program – Certain Assisted living facilities offer rooms or apartments, meals, assistance with personal care and medications. There are three programs to assist low income elders interested in residing in assisted living
Protective Services – The case manager offers social service support and comfort to those elders who suffer from physical or psychological abuse, financial exploitation, and abandonment or neglect. The elder who can no longer care for him/herself thus becoming a self-neglect client could be eligible for a myriad of services that a case manager would provide and monitor.
The Rhode Island Pharmaceutical Assistance to the Elderly Program (RIPAE) pays sixty percent toward certain prescriptions for clients who are 65 years of age or older and who meet specific income guidelines.
